The embouchure for clarinet is much more demanding than for sax (I majored in performance for both)... so if she can do clarinet, you can do alto! As long as it's fully healed, nothing will happen. However, to get the correct embouchure, you do need to take out the stud and put in a filler. And just remember to keep it clean! If I were to get a labret piercing ( I play clarinet, sax, flute and piano, by the way) I would play in five minute incriments to get used to playing with an extra something in the way. at first, yes. it will probably hurt a lot, too.
